Insurance Information
We accept most major insurance plans. If you have an HMO Insurance, it is your responsibility to bring the necessary authorization form from your PCP (Primary Care Physician) office to your appointment. If you need any information regarding insurance or billing question please contact our billing department at 586-582-0864.
Appointments
Please call the office to make an appointment at 586-582-0760. If you need to reschedule or will not be able to keep your appointment we appreciate notification at your earliest convenience. This will allow us to provide prompt service to others in need. We make a sincere effort to adhere to our appointment schedule and appreciate your patience if we are late due to patient emergencies or surgery.

Prescription Refills
For your convenience, most prescription refill requests can be responded to during office hours. To ensure that you have enough medicine please monitor it carefully, anticipate your needs and call your pharmacy at least two days before your supply of medicine is depleted.
Unfortunately we are unable to respond to your prescription requests after hours.
Fees & Payments
We make every effort to decrease the cost of your medical care. Therefore, we request payment for all office services at the time they are rendered unless prior arrangements have been made. We accept cash, checks, MasterCard, Visa, and Discover for your convenience. However, payment is the patient’s responsibility. We will help in any way we can to assist you in handling claims.
